Tax Organizer     

A tax organizer can facilitate the process of pulling together your tax information. This basic tax organizer is designed for new clients and allows you to enter your information right on the screen. You can then print the completed tax organizer and fax, mail it to the office, or bring with you to your appointment.

Current clients looking to utilize a tax organizer should contact the office to request a "proforma" tax organizer that includes prior year information and carryover data.

Client Data Sheet

New and existing clients who do not wish to utilize a tax organizer can find our client data sheet below. We request a new client data sheet be filled out each year to assist us in preparing your tax return. Please bring the completed form with you to your tax appointment. If you do not have a printer, we have paper copies available at the office that you can pick up at your convenience.

There are two options below: The first allows you to complete the form electronically and then print the completed form. The second allows you to print a blank form to be completed on the paper version.
